The carbon typescript copy of Gene Kelly's "Payroll Change Request" form, for his job as "Consultant," "On Payroll Date" "6/21/81," noted under "Special Instructions" as "Transferred from OFTH ["One from the Heart"] Payroll," with carbon typescript copy signatures from Zoetrope Studio President Robert Spiotta, and Zoetrope Vice President and General Manager Alex J. Carter, with the date "7-22-81" adjacent to Carter's signature.

Gene Kelly worked alongside choreographer Kenny Ortega as an uncredited consultant on the film, most notably as dance consultant on Terry Garr and Raúl Juliá's highly stylized tango sequence to Tom Wait's instumental track "The Tango/Circus Girl." Kelly was reported to have disagreed with Coppola over the story the dance was meant to portray, suggesting a different interpretation of the dance. Coppola released the film in 1981 with his preferred sequence of the dance but restored Kelly's sequence for the film's 2003 restoration and home media release.
